One formula covers every salary: 5% of the monthly basic, clamped to the ₱10,000–₱100,000 band, half deducted from the employee and half shouldered by the employer. Reference points:

Common questions.

The premium rate is 5% of the monthly basic salary — the Universal Health Care law's ceiling rate, unchanged for 2026 — split equally: 2.5% employee, 2.5% employer. The income floor is ₱10,000 and the ceiling ₱100,000, so the total premium runs from ₱500/month (₱250 each) to ₱5,000/month (₱2,500 each).

Clamp your monthly basic salary between ₱10,000 and ₱100,000, multiply by 5%, then split in half. Example: at ₱20,000 the premium is ₱1,000 — ₱500 deducted from the employee, ₱500 paid by the employer. Below ₱10,000 the premium stays ₱500; above ₱100,000 it stays ₱5,000.

5% is the final scheduled rate under the UHC law (RA 11223) — the yearly step-ups ended when the rate reached 5% and the ceiling ₱100,000. Adjustments beyond this require new issuances, so always check the latest PhilHealth circular.

The security agency, as the employer of record, deducts the employee half and remits the full premium. Under D.O. 174 the client bill rate must be enough to cover statutory contributions including PhilHealth — a bill rate that ignores it is quietly eating the agency's margin.
